There is a very brief review of goals of care and guiding conversations with patients and their chosen Health Care Surrogate. The Surrogate needs to be included as a listener so they have an understanding of the patients wishes. The HCS is responsible to be the patient's voice, when the patient can no longer represent themselves. The book reviews 4 or the 5 life sustaining treatments: Feeding Tubes, Dialysis, Mechanical Ventilation, and CPR. I have some critiques that I leave regarding the information in the brief booklet.
