Four molar implants in one patient — and a real-time masterclass on the thing that makes new surgeons nervous: what to do when something bleeds. This live planning session walks attendees through the full quadrant while teaching the decisions and the save-your-day techniques along the way.
The case-planning fundamentals:
• Approaching a four-molar case — one at a time vs. batching your osteotomies, and why starting with #3 lets you learn the bone and course-correct on the rest
• Incision choice on a wide, high-KG ridge — when midcrestal is fine and when to lingualize
• Envelope flap vs. distal release when there's no distal tooth — visibility vs. healing, and why a clean flap matters more than a small one
• When to engage the sinus floor and when to stay short — and why a 5x10 is easier to repair down the road than a 5x16
• Restoratively driven placement over buccal bone loss — stepping to a 5x8 to stay straight instead of tipping lingual
• Harvesting bone with a hockey-stick extension and safe scraper if you ever need to graft
Then the part worth saving:
• Soft tissue bleeds — compression, anesthetic with vasoconstriction, clamping with a hemostat (and the timer trick so you don't necrose the tissue), a battery-operated cautery unit, and TXA if you have an IV in
• Bone bleeds — the "barbecue" technique (dry bur on bone to cook the vessel closed), crushing with the back of a mirror, and bone wax
• A nicked vessel in the osteotomy — the guide-pin trick to plug the hole and control the bleed until the implant seats
And a great teaching thread on autonomy: whether to do your hardest implant first (fresh and caffeinated) or build confidence on the easy ones — and why, in the operatory, "you're the quarterback, we're just the guardrails."
Bring your cases and run them by Dr. Briskie or the CSI faculty anytime.
