Why did Deepwater Horizon’s blowout preventer fail to seal the Macondo well? Follow the failed cement barrier, conflicting negative-pressure test, rising influx and hidden pipe-buckling mechanism examined by the U.S. Chemical Safety Board. The CSB concluded that the blind shear ram likely activated after the first explosions, but could not seal around displaced drill pipe. Earlier investigations differed on activation timing. The final barrier’s failure was one part of the disaster’s chain of failures. Original explanatory 3D reconstructions, synthetic English narration and original music are combined with verified Coast Guard/Navy archive. The reconstructions are not historical recordings or validated simulations. Fire imagery shows aftermath; investigation, recovery and response footage is from later dates. Thumbnail artwork is an original conceptual illustration informed by official investigation reports and U.S. Coast Guard reference imagery. It compresses time, scale and depth for the cover; it is not an archival photograph or a dimensionally exact engineering model.
