Hello, I am Dr. Euiryong Lee, Oral and Maxillofacial Surgery, Chung-Ang University Hospital, Seoul. Today I share the case of a 31-year-old man who came to me asking for one thing: to be restored exactly to how he looked before his surgery. This is a revision bimaxillary (double jaw) case combined with total jaw reconstruction. My sincere thanks to the patient for allowing me to share it to help others facing the same struggle. BACKGROUND In December 2024 the patient underwent double jaw surgery and facial contouring at another clinic. When he came to me, his main complaint was that his mouth and chin had been pushed too far backward, and he strongly wished to be returned to his exact pre-surgery state. CT FINDINGS (at presentation) An anterior open bite had developed. The condyle-bearing (proximal) segment was rotated forward. The mandibular angle had been resected and folded forward, with significant bone loss from infection and osteolysis at that site. The square-jaw contour had collapsed inward, sharply reducing the bigonial width. Superimposing the pre-original-surgery scan (T0) over the presenting scan (T1) clearly revealed the angle bone loss and segment rotation. SOFT TISSUE As the chin receded, a step-off appeared between the upper and lower lip. The chin looked small against a high nose, and the lower lip sat behind the upper lip, a clear loss of balance. SURGICAL PLAN (reverse-engineered) Because the patient wanted an exact return to T0, the plan was clear: superimpose T0 and T1 and reverse-calculate the path back to T0. Maxilla midline shift 1 mm to the left Maxilla downward movement 4.2 mm Counterclockwise rotation 1.2 mm Maxillary advancement 3.5 mm After digital simulation of the maxilla moving down, forward and counterclockwise, I designed the mandibular angle reconstruction. Lowering the maxilla by 4.2 mm with counterclockwise rotation opens a gap that needs block bone, so iliac (pelvic) bone, needed anyway, was harvested and used both to support the down-grafted maxilla and to rebuild the mandibular angle, with a patient-specific guide and a PCL plus autogenous bone framework. TECHNIQUE AND DIFFICULTY To close the anterior open bite, the mandible itself had to counter-rotate. Doing this with infection-weakened bone, while fixing the segments and reconstructing the angle at the same time, made this a very demanding case. A bone gap on the left side was filled with the remaining graft to restore bony continuity. The reconstruction was completed in two stages; at the second stage the mandibular pin was removed and the contour augmented slightly further. RESULTS (about one year later) The maxilla moved down and forward and the occlusion normalized. The lost mandibular angle was rebuilt to a full length, and the entire lower jaw, angle through chin, was reconstructed. The left-side gap healed with restored continuity. A weak lower face became a defined, masculine jawline, and the upper-to-lower lip relationship and overlap improved. The short lower face, previously out of proportion with a long midface, was lengthened into a far more harmonious, balanced profile. Reassuringly, no muscle dysfunction occurred. Comparing T0 and T2, the result is not identical but strikingly close to his original face. The patient told me he is now genuinely happy and free to stop worrying about it. TAKE-HOME MESSAGE For anyone who wants their old face back after jaw and contouring surgery, revision bimaxillary surgery with total jaw reconstruction can return you remarkably close to your pre-surgery self. One Surgery, Zero Worry: precise diagnosis and patient-specific planning so a single surgery can lift the worry.
