This visual glitch/bug is more common than you think. Is it fixable? Yes, just save and restart; no big deal. Is it playable? Yes, I just did it. It happens along highways as well, and being that high is deadly not just for the companions, but for you as well. I left mine on stay, so I didn't have to parkour and make crazy jumps when they get stuck along the sides of the walls. They don't register the ledge or the ground beneath them during this glitch, so they run off every time. On the Highways, it is the same; sometimes you don't see the holes and fall through. On survival, if you fail to repair them when down or heal them, you have to go back to the settlement you assigned them to. If you didn't assign them, go back to where you first met them; you have to go back to get them as a follower again.
