A viewer asked me to respond to Dr. Anthony Jay's video arguing that hard plaque is the real danger, soft plaque is overblown, and the calcium score is the number to trust. So I watched it with you, claim by claim, then went to the papers he cited and a few he didn't. The short version: a calcium score is real information, but it's the tip of an iceberg, and the research on calcium density and soft-plaque imaging tells a more interesting story than "hard good, soft bad." The back half is a proper walk through three papers — MESA calcium density, an outpatient CT angiography study, and SCOT-HEART — that get at the actual question: is the calcium itself the problem, or is it just marking how much total plaque you've been carrying?
