In this video, Dr. Donald Ozello demonstrates the Hold-Relax Agonist-Contract (HRAC) technique of Proprioceptive Neuromuscular Facilitation (PNF) stretching. This advanced flexibility method uses the body's neuromuscular reflexes (autogenic and reciprocal inhibition) to achieve immediate gains in range of motion.
📋 Clinical Context: Why HRAC Works HRAC is superior to static stretching because it "hacks" the muscle spindle and Golgi tendon organ response:
Phase 1 (Hold-Relax): Isometric contraction of the tight muscle (Hamstring) fatigues the motor units and triggers autogenic inhibition.
Phase 2 (Agonist-Contract): Active contraction of the opposing muscle (Hip Flexors) triggers reciprocal inhibition, forcing the hamstring to relax further.
Result: Immediate, pain-free increase in hip flexion.
Technique Breakdown:
Patient Position: Supine, leg passively raised to the point of mild tension.
Step 1 (Isometric): Patient pushes into extension (75-95% effort) for 10 seconds against the doctor's resistance.
Step 2 (Relax & Contract): Patient exhales, relaxes the hamstring, and actively contracts the hip flexors (Quads/Psoas) to pull the leg deeper into the stretch.
Step 3 (Hold): The doctor stabilizes the new range for 10-15 seconds.
Repetitions: Perform 3-5 cycles per leg.
