Most of the attempts died in the first room, not because it's the hardest or anything, but because you can sometimes just take an 80 damage rev rocket when dealing with the pain elementals and it just feels bad. Most other attempts were silly mistakes like getting faked out by a cyberdemon and taking a rocket to the face somewhere. The pain elemental flood after the main fight in the plasma rifle room is really annoying, it's entirely possible to take 0 damage or just face rocket because 5 pain elementals flood in at once and start spitting immediately. The archvile bfg room in this is ugly, but it's fine.
