Bursa or Bursae of the Shoulder Joint and Region In this video, we discuss the Bursae of the Shoulder Joint and surrounding region, small fluid filled sacs that play an important role in reducing friction between moving structures around the shoulder. What is a Bursa A bursa is a small, fluid filled sac lined by synovial membrane, located at points where friction commonly occurs between tendons, muscles, bones, and skin. Bursae help facilitate smooth movement and reduce mechanical stress on surrounding structures. Major Bursae of the Shoulder Region The Subacromial Bursa, sometimes considered together with the Subdeltoid Bursa as one continuous structure, lies between the acromion and deltoid muscle superiorly and the supraspinatus tendon and joint capsule inferiorly, playing a key role in shoulder movement and often implicated in impingement syndromes. The Subscapularis Bursa lies between the subscapularis tendon and the neck of the scapula, and typically communicates with the shoulder joint cavity. The Subcoracoid Bursa is located beneath the coracoid process and may communicate with the subscapularis bursa. Other smaller bursae, such as those related to the coracobrachialis and infraspinatus tendons, may also be present. Clinical Relevance Inflammation of these bursae, known as Bursitis, most commonly affects the Subacromial or Subdeltoid Bursa, often associated with repetitive overhead activity, rotator cuff pathology, or shoulder impingement. This can result in pain, swelling, and restricted shoulder movement, particularly with abduction. Diagnostic and Clinical Importance Understanding the location and relations of these bursae is essential for accurately diagnosing shoulder pain, differentiating bursitis from other causes such as rotator cuff tears or joint pathology, and guiding procedures like corticosteroid injections.
