Breaking the Silos Integrating Palliative Care, Geriatrics & Rehabilitation for Older People Living

This webinar, organised by the EAPC Reference Group on Aging and Palliative Care in collaboration with the EAPC Rehabilitation in Palliative Care Task Force and the EU NAVIGATE project (Horizon Europe), brought together international experts to explore integrated approaches across palliative care, geriatrics and rehabilitation. It focused on improving care and support for older people living with frailty and cancer, highlighting innovative navigation interventions to enhance access and quality of life.
