To all students who are eligible to proceed to Higher Education, SAMAR STATE UNIVERSITY will start conducting its admission activity for Academic Year 2022 – 2023 on February 17. Interviews will follow on May 2 – 31, 2022. The application for admission of student-applicants will be done online through a registration link posted at SSU – Student Welfare and Development Services and Official Facebook pages of the different colleges and external campuses. Please bring the following requirements upon enrollment registration: • For NEW STUDENTS Grade 11 Form 138 or its equivalent (for Grade 12 on-going students) Grade 12 Form 138 or its equivalent (for Grade 12 graduates) 4 pcs Passport size photo with name tag *Grade 11 Form 138 will only be used for screening purposes only, when qualified, Grade 12 Form 138 will be submitted to the University Registrar • For SHIFTERS Shifting Form (provided by the Registrar) Evaluation of Grades 4 pcs Passport size photo with name tag • For RETURNEES Evaluation of Grades Permit from the College Dean (to be processed by SWDS) 4 pcs Passport size photo with name tag For more information, clarifications, and inquiries please contact the official Facebook page of SSU – SWDS or at 0977-361-6144 on official hours from 8:00AM to 5:00PM. For incoming Masteral or Doctoral students: • Official Transcript of Records; • Honorable Dismissal from the College/University last attended; • Copy of Graduate Thesis (for Doctoral students); • Permit to Study (for Government employees); • Photocopy of Certificate of Live Birth from NSO; • Three (3) passport-sized photos; • One (1) orange expandable envelope and one (1) brown envelope; • Medical certificate; and • Application for Admission (downloadable) B. Continuing Students • Please send the following details for your enrollment confirmation: 1. Name; 2. Program; 3. Contact Number; and 4.
