Matt has been dealing with numbness in both hands and arms for approximately two years. It began around the time he injured his elbow while play wrestling with his brother. Urgent-care X-rays reportedly showed no fracture, but his work at a doggy day camp meant larger dogs were continually pulling on the injured arm before it had fully recovered. Although the elbow eventually improved, the neurological symptoms did not. Within several months, Matt noticed numbness affecting both arms and hands, along with episodes where his hands would curl and contract. Two years later, he was still wearing an elbow brace. Now, with a one year old baby at home, Matt wants to understand what is going on and improve his ability to comfortably use his arms and hands. In Part 1 of Matt’s journey, Dr. Rahim performs a detailed Gonstead evaluation and reviews both his full-spine weight-bearing X-rays and elbow films. The spinal films reveal several important structural findings, including previous tailbone injuries, sacral and pelvic misalignment, a transitional L5 vertebra fused with the sacrum, and multiple compensatory changes extending into the thoracic and cervical spine. Of particular interest are the multiple upper thoracic compensations and findings involving C6. The nerves supplying the upper extremity originate from the cervical region, but rather than focusing on one vertebra or assuming one finding explains Matt’s symptoms, Dr. Rahim evaluates how the entire structure is functioning together. His examination identifies findings at S3, T9, T2 and C6. Dr. Rahim also evaluates and adjusts the shoulder and an elbow finding involving the distal humerus. This case demonstrates an important principle of the Gonstead System: Don’t simply chase the location of the symptom. Examine the foundation, identify the relevant structural findings, determine what may be primary versus compensatory, and make specific corrections based on the examination.
