The CMS ACCESS Model creates a new opportunity for ACOs to connect beneficiaries with technology-enabled care while advancing clinical outcomes and value-based care performance. Join an ACO leader and approved ACCESS organizations for a practical discussion on how ACO leaders can strategically incorporate ACCESS into their broader care-management and performance strategies. The discussion will explore how ACOs can use ACCESS to extend resources available to patients and practices, support priority populations, and potentially improve outcomes and performance. The speakers will also share innovative approaches for coordinating across multiple ACCESS organizations serving patients with comorbidities, while keeping the patient’s physician and care team at the center and avoiding duplicative services, conflicting care plans, and fragmented care.
