We begin by dividing this drug class into two main categories based on their site and mechanism of action in the collecting ducts of the nephron:
• Aldosterone antagonists: Spironolactone and Eplerenone—block mineralocorticoid receptors, reducing sodium reabsorption and potassium excretion
• Epithelial sodium channel blockers (ENaC inhibitors): Amiloride and Triamterene—directly block sodium channels, preventing sodium reabsorption and potassium loss
The lecture reviews key indications including heart failure with reduced ejection fraction (HFrEF), resistant hypertension, primary hyperaldosteronism, hypokalemia, and cirrhosis with ascites. You’ll also learn about off-label uses such as hormonal acne and polycystic ovarian syndrome (PCOS) with spironolactone.
