Stiff-Person Spectrum Disorders: Autoimmunity, Diagnosis, and the Future of Treatment DOI: 10.5281/zenodo.16113887 ABSTRACT This episode explores Stiff-Person Spectrum Disorders (SPSD)—a rare, complex group of progressive autoimmune neurological conditions that cause debilitating muscle rigidity, painful spasms, and severe anxiety. From classic Stiff-Person Syndrome (SPS) to life-threatening variants like progressive encephalomyelitis with rigidity and myoclonus (PERM), we examine the underlying breakdown in GABAergic inhibition and the emerging evidence of both B-cell (antibody) and T-cell involvement. We explain the diagnostic triad, EMG findings, and the antibody profiles that define subtypes (GAD65, amphiphysin, glycine receptor, and others).
