In 2007, a bar owner called Brian Juan on a Saturday and told him not to come in. They were replacing the dueling pianos with a mechanical bull. He was 33. Six weeks later he was sitting in a chemistry class.
Today he is Dr. Brian Juan, and he runs Metronome Family Medicine, a solo Direct Primary Care practice in Longmont, Colorado, built around midlife vitality and longevity. He opened on June 1, 2026. Three months later he has 114 members and a small business loan he has never touched.
Host Dr. Maryal Concepcion talks with him about the part most people skip: what it actually took to leave. Not the inspiration, the mechanics. The nine years he stayed in a corporate system out of real financial fear, carrying student loans, a foreclosure, and a bankruptcy from medical school. The savings buckets system that finally changed the math at home. The phone call to another DPC physician who answered his own phone and spent three and a half hours building a pro forma with a total stranger.
They also get into the music, because you cannot tell this story without it.

ABOUT DIRECT PRIMARY CARE
Direct Primary Care is an alternative to insurance driven healthcare. A patient pays their doctor a low monthly membership and in return builds a lasting relationship with a doctor who is actually available to them. No insurance billing, no prior authorizations, no seven minute visits.
