Steps taken: Have either Cernan or Eisele get caught during their individual stealth sequences Have the same character who got caught during the stealth sequence get caught when they investigate Hydroponics (they sustain a wrist injury leading to their death during the individual stealth sequence) At the scanner sequence, choose to send the both Eisele's or both Cernan's through the scanner Voila, death spiral!
