He notes that major psychotic disorders can complicate or rule out a BPD diagnosis in some diagnostic systems, which makes careful assessment important. In BPD, psychotic symptoms are often described as mild, temporary, and related to hallucination-like experiences. They may become stronger during periods of intense stress, emotional crisis, trauma activation, or PTSD symptoms. These experiences can also be linked to greater distress, increased suicide risk, and a higher need for hospitalization. Dr. Fox explains that hallucinations in BPD can vary. Some may be more neutral, less frequent, and easier to control, while others may be more negative, frequent, distressing, and harder to manage. For treatment, research is mixed. Some studies show benefit from atypical antipsychotic medications, while others show limited or no benefit.
