The Deadly Risk After a Plant Shuts Down | Facility Management During decommissioning work at a West Virginia industrial facility, a chemical transfer released toxic hydrogen sulfide. Two workers died, others were seriously injured, and emergency responders were also exposed. The incident highlights a critical Facility Management lesson: shutting down a facility does not remove its hazards. Decommissioning creates a new operation—with different processes, changing site conditions and risks that may not have existed during normal production.
