What happens when a refinery shuts down, but the hazards remain active?
On April 26, 2018, a planned shutdown at the Husky Superior Refinery led to a major explosion, vessel failure, flying debris, an asphalt fire, worker injuries, and the evacuation of nearby residents.
This video examines how changing pressures, worn equipment, missing steam barriers, incomplete purge arrangements, weak shutdown procedures, and gaps in process knowledge allowed air and hydrocarbons to mix inside the fluid catalytic cracking unit.
