Rossetti SC, Dykes PC, Knaplund C, et al. Real-time surveillance system for patient deterioration: a pragmatic cluster-randomized controlled trial. Nat Med. 2025.
When nurses sense that a patient is deteriorating, their documentation changes before the vital signs do. More frequent checks, additional notes, entries outside expected intervals. CONCERN treats that surveillance behavior as a measurable signal and uses it to flag patients at risk.
In a multi-site pragmatic cluster-randomized trial across four hospitals, the system was associated with a 35.6% reduction in the instantaneous risk of in-hospital death and an 11.2% decrease in length of stay.
