🚨 A child with growth failure, polyuria, rickets, and generalized proximal tubular dysfunction? Think NEPHROPATHIC CYSTINOSIS! In this high-yield nephrology board review, we cover the genetics, pathophysiology, clinical presentation, diagnosis, and treatment of cystinosis—a lysosomal storage disorder that causes progressive kidney disease. 🧠 1. PATHOPHYSIOLOGY Autosomal recessive mutations in CTNS cause defective cystinosin, the lysosomal cystine transporter. Cystine accumulates inside lysosomes, forming crystals and damaging multiple organs. The kidneys are often affected early, particularly the proximal tubules. ⚡ 2. CLASSIC RENAL PRESENTATION: FANCONI SYNDROME Generalized proximal tubular dysfunction causes urinary loss of: Glucose despite normal blood glucose Phosphate, causing hypophosphatemia and rickets Bicarbonate, causing proximal RTA Amino acids and uric acid Sodium, potassium, and water Clinical findings include polyuria, polydipsia, dehydration, growth failure, and metabolic acidosis. 🔬 3. TYPES OF CYSTINOSIS Infantile nephropathic: Most severe; early Fanconi syndrome and progressive CKD. Juvenile nephropathic: Later and often milder renal presentation. Ocular non-nephropathic: Primarily corneal involvement without the classic early renal phenotype. 👁️ 4. EXTRARENAL MANIFESTATIONS Cystine accumulation may affect: Cornea: crystals, photophobia, and later visual complications Thyroid: hypothyroidism Pancreas: impaired glucose regulation or diabetes Muscles: weakness and wasting Other organs, particularly in advanced disease 🩺 5. DIAGNOSIS Measure leukocyte cystine concentration. Confirm with molecular testing for CTNS variants. Slit-lamp examination can identify corneal crystals, although they may be absent early in infancy. Evaluate electrolytes, bicarbonate, phosphate, kidney function, growth, and evidence of Fanconi syndrome. 💊 6. TREATMENT Cysteamine reduces intracellular cystine accumulation and can delay progression of organ damage. Replace fluids and electrolytes; treat acidosis, hypophosphatemia, and vitamin D-related bone disease as indicated. Monitor growth, thyroid function, eye disease, and other organ involvement. Advanced kidney failure may require dialysis or kidney transplantation. Transplantation treats the renal failure but does not eliminate cystine accumulation in other organs. 🎯 HIGH-YIELD BOARD PEARLS Infantile nephropathic cystinosis is a classic cause of Fanconi syndrome in children. CTNS mutation causes defective lysosomal cystine transport. Expect glucosuria with normal serum glucose, phosphaturia, bicarbonate wasting, and growth failure. Cysteamine treats the underlying cystine accumulation; kidney transplantation does not cure the systemic disorder. 📚 Educational content for nephrology fellows, residents, and clinicians. #Cystinosis #FanconiSyndrome #ProximalRTA #CTNS #Tubulopathy #PediatricNephrology #KidneyDisease #NephrologyBoardReview #MedicalEducation #Nephro HighYield
