A capsule camera can only diagnose what it can see.
That simple principle lies at the heart of one of the most important, and sometimes underestimated, aspects of capsule endoscopy: bowel preparation.
In this Gut Check Lab conversation, Professor Owen Epstein, Consultant Gastroenterologist, and Tim Rayne explore why effective bowel cleansing is so important for both small bowel and colon capsule endoscopy, and what patients and capsule teams can do to improve the quality of the examination.
Unlike conventional endoscopy, a capsule cannot wash the bowel, suction away debris or ask for a second look. Once swallowed, the capsule travels independently through the gastrointestinal tract. If the view is obscured by residual material, bubbles or poor cleansing, potentially important abnormalities may be more difficult to identify.
We explore:
• Why bowel preparation matters so much in capsule endoscopy
• The principles behind dietary restriction and bowel cleansing
• Why the timing of preparation can be as important as the preparation itself
• The role of split-dose bowel preparation
• How bubbles, debris and intestinal contents can interfere with mucosal visualisation
• Why colon capsule endoscopy presents an additional challenge — the capsule must not only see clearly, but also complete its journey through the colon within the recording period
• The role of boosters and prokinetic strategies in supporting capsule progression
• Why constipation and previous poor bowel preparation should be identified in advance
• How better patient education can improve preparation and the overall quality of the examination
We also introduce a simple way of thinking about capsule preparation:
CLEAN — SEE — COMPLETE
Clean: remove material that may obscure pathology.
See: create the clearest possible view of the bowel lining.
Complete: help the capsule reach the end of the examination within the available recording time.
And underpinning all three is another essential ingredient: the patient needs to understand why each step matters.
At the Gut Check Lab, we believe good preparation begins well before the patient takes their first dose of bowel cleanser. Clear instructions, accessible audiovisual education and an understanding of the purpose behind each stage can help transform bowel preparation from a list of instructions into an active partnership between the patient and the capsule team.
Because ultimately, however sophisticated the capsule technology becomes:
Poor preparation means poor pictures, and poor pictures can mean a poorer examination.
