A 25-year-old woman arrived at the hospital with a platelet count of just 10,000 and altered consciousness. What initially looked like severe thrombocytopenia turned out to be TTP (Thrombotic Thrombocytopenic Purpura), a life-threatening medical emergency. The peripheral smear revealed schistocytes (broken red blood cells), helping raise suspicion for TTP. With early recognition and rapid intervention, plasma exchange was started within 3 hours, and her platelet count improved from 10,000 to 70,000 within 24 hours. In TTP, deficiency or inhibition of ADAMTS13 allows unusually large von Willebrand factor (vWF) multimers to accumulate, leading to widespread tiny blood clots that can damage vital organs such as the brain and kidneys. This case is a powerful reminder that in emergencies like TTP, time is medicine. Early suspicion. Urgent referral. Timely treatment. Most importantly, teamwork. #TTP #ThromboticThrombocytopenicPurpura #Hematology #BloodDisorders #LowPlatelets #MedicalEmergency #PlasmaExchange #ADAMTS13 #Hematopathology #EmergencyMedicine #MedicalEducation #Healthcare #Medicine #patientsafety
