Can low stomach acid be autoimmune? In autoimmune gastritis, the immune system attacks the stomach cells that make acid, targeting the proton pump, the same molecule heartburn pills switch off. And in one study of people with Hashimoto's and other autoimmune thyroid disease, those who carried the antibody that marks this attack had less than half the odds of reporting heartburn.
If you've wondered what causes low stomach acid, how you'd know if you have it, or why low stomach acid symptoms often aren't heartburn, this is the mechanism, step by step.

What we don't know: both thyroid studies looked at patients at a single point in time at one center each, and the heartburn finding came from a questionnaire, not a measurement of acid or reflux. Neither study measured the lower esophageal sphincter, so the "less acid, less burn" explanation is plausible, not proven. In the Athens study, only antibody-positive patients had an endoscopy, so the finding that all five stomach tumors were in that group can't be read as a fair comparison. Carrying the antibody doesn't mean someone has atrophic gastritis.
