🚨 Depression in dialysis patients can be challenging to diagnose and treat. Which antidepressants are preferred, and which require extra caution in ESRD? 🧠 HIGH-YIELD MANAGEMENT: Screening: Use PHQ-9 and clinical assessment; evaluate overlapping symptoms from uremia, anemia, sleep disorders, pain, and psychosocial stressors. Nonpharmacologic therapy: Cognitive behavioral therapy, social support, and addressing dialysis-related burdens. Preferred options: SSRIs such as sertraline are commonly considered, with individualized dosing and monitoring. Citalopram and escitalopram: Use caution because of dose-dependent QT prolongation, particularly in patients with cardiac disease or electrolyte abnormalities. MAO inhibitors: Generally not first-line because of significant drug and food interactions and risks such as hypertensive crisis and serotonin syndrome. Other precautions: Review renal dosing, drug interactions, bleeding risk with SSRIs, hyponatremia, QT prolongation, and accumulation of active metabolites. Avoid abrupt discontinuation. Safety first: Assess suicidal thoughts and arrange urgent evaluation when there is an immediate safety concern. 🎯 BOARD PEARL: No antidepressant is universally contraindicated solely because a patient has ESRD. Selection depends on renal pharmacokinetics, comorbidities, interactions, and adverse-effect risks. Citalopram and escitalopram are not automatically prohibited, but QT risk deserves particular attention. 🔥 TAKE-HOME MESSAGE: Treat depression in ESRD with individualized pharmacotherapy, psychosocial support, and careful monitoring. #Nephrology #ESRD #Dialysis #Depression #Antidepressants #Sertraline #NephrologyBoards #MedicalEducation
