When Someone With Dementia Refuses Medication: What Caregivers Should Do

You come in and say, “Mom, you have to take these meds.” And she says, “No. I’m not taking that.” If this is happening in your home, you are not alone. In this episode of Straight Talk with Dr. Erik, I talk about why medication refusal happens in dementia and why caregivers should not assume it is “just behavior.” Sometimes there is a physical reason. Maybe the person has trouble swallowing. Maybe the medication causes nausea, dizziness, stomach discomfort, or another side effect. Sometimes there is a cognitive reason. The person may not believe they are sick or may not understand why the medication is needed. Sometimes there is a psychological reason. Paranoia, fear, distrust, or confusion can make a person believe the medication is harmful. And sometimes the issue is environmental. A rushed caregiver, a noisy room, a change in routine, or a new person giving the medication can all make refusal worse. In this video, I walk through practical caregiver strategies, including: • finding out why the refusal is happening • speaking with the doctor about side effects • asking whether a medication is still necessary • making medication time part of a calm routine • using simple language instead of arguing • avoiding medication “battles” • asking about alternate forms like patches, liquids, or sprinkles when appropriate • involving the pharmacist and medical team • tracking patterns around refusal This is not about forcing someone. It is about understanding what the behavior may be communicating and working with the right medical professionals to make the process safer and calmer.
