Every field has its own O-ring — something that shows up as a little off, a little odd, a little worse than it should be, and doesn't get anyone hurt. Yet. And somebody in the room quietly says, "It's probably fine." Go back to the first scene. A group of engineers, on a phone line, saying they can't prove it's unsafe but they know it is. What do you do when you're the one on the line? And what do you do when you're the one on the other end? This isn't a video about rockets exploding. It's a video about warnings that were heard, written down, tested — and then set aside, twice, for the same kind of reason. The astronauts of Challenger and Columbia didn't die because nobody knew. They died because knowing wasn't enough. Somebody has to listen. Stay to the end — because the same pattern shows up again at the end of this story, in a place you wouldn't expect, and the last question in this video is the one that matters most for the next flight.
