The Health Foundation’s NHS Productivity Commission report reframes healthcare productivity as a challenge of system design and capability rather than simply demanding greater effort from overworked staff. Although the diagnosis accurately identifies how constant organisational churn, excessive bureaucratic compliance, and fragmented priorities create a self-perpetuating cycle of poor performance, the report's prescriptions require much greater operational clarity regarding decision rights, accountability, and the complexities of transitioning resources. Furthermore, large-scale initiatives such as digital transformation and artificial intelligence cannot be viewed as instant productivity fixes because technology only delivers genuine value when it successfully alters workflows and releases usable capacity. Ultimately, sustainable healthcare improvement depends on deliberate subsidiarity, rigorous benefits realisation, and difficult decisions about power, resources, and which outdated activities the service must stop doing.
