It's been over 3 years since the release of ChatGPT, yet in clinics across the globe we're still faxing medical records, most EHR vendors only support FHIR reads, and we haven't seen anyone hand an entire implementation guide (IG) off to an agent to support (as far as we know). Building in healthcare today you need to be a time traveler, simultaneously living in the past, present, and future. In my talk I'll share the practical challenges and opportunities we hear from developers and researchers every day from building with FHIR IGs and LLMs to dealing with messy EHR data and context windows. I'll share best practices for developers navigating high-stakes healthcare use cases ensuring determinism, auditability, and accuracy. Through case studies and examples from across the industry, we'll look at what's working for healthcare builders right now, what isn't, and where patients and providers most need our support.
