Errata:
Question 3 should be [B] -- Impaired left ventricular filling resulting in decreased left ventricular stroke volume
Question 19 should be [C] -- staph aureus.
Brief Explanations:
1. [A] Sacroiliitis, eye symptoms (uveitis), male should point you towards Ankylosing Spondylitis. Appropriate initial treatment would be NSAIDs. 2nd line - corticosteroids. 3rd line - TNF-alpha inhibitors.
2. [A] Barrier protection is the only method listed here to prevent the spreading of STIs.
3. [B] Cardiac tamponade (low BP, high HR, JVD, muffled heart sounds) leading to cardiogenic shock; blood pools in pericardium which prevents both the left from filling.
4. [A] Abdominal pain + elevated BP + hypertonic uterus = Placenta abruption
5. [B] When you still include patients who were lost to follow up or didn't complete the trial on the study arm, that is in agreement with the principle of Intention to Treat. (just memorize it unfortunately)
6. [B] Carpal tunnel symptoms and hyperglycemia with disproportionate digits and limbs should make you think of acromegaly. Best initial test is IGF-1 levels and glucose suppression test.
7. [D] Holosystolic left sternal border murmur with no central cyanosis at birth is indicative of VSD.
8. [E] Hypertensive changes, fatigue, muscle weakness (low [K+]), and adrenal mass is consistent with aldosteronoma. High Aldosterone = metabolic alkalosis, hypernatremia, hypokalemia.
9. [B] Previous rhinoplasty with whistling noise on inspiration is most likely due to septal hematoma that lead to perforation.
10. [D] Physical exam findings (bruits by umbilicus and persistent HTN) are consistent with renal artery stenosis. You need to first visualize in order to confirm the diagnosis before any further intervention.
