Rh incompatibility is still a major cause of mortality and morbidity in India. If undetected it causes havoc in pregnancy and may lead to abortion, hydrops details or intra uterine death. Babies develop pathological jaundice in first few hours of life which progresses very fast and if not treated properly in time it may lead to permanent brain damage due to kernicterus. Therefore it's necessary that it should be diagnosed early in pregnancy and Anti D Immunoglobulin must be given early in pregnancy and the second dose should be given within 72 hrs of delivery to the mother. Babies of Rh negative mothers should be kept in NICU under strict monitoring and Hb and S. Bilirubin must be measured every 6 hrly initially. If the level is not rising fast then phototherapy and IVIG is the treatment of choice. In case the levels of bilirubin is rising very fast and Hb is falling then usually this is the baby who needs Exchange Transfusion. Exchange transfusion is the process where the positive blood of baby is replaced by negative blood group. The process is explained in my next video. These treatments if given timely the outcome is very good and the babies develop normally and lead a normal healthy life.
