The 2024 SELECT trial randomized over 17,000 adults with overweight/obesity and pre-existing cardiovascular disease — none of whom had diabetes — to semaglutide 2.4 mg weekly or placebo. The primary outcome wasn't weight loss. It was major adverse cardiovascular events (MACE) — heart attack, stroke, and cardiovascular death. The semaglutide group experienced a 20% relative reduction in MACE that was statistically separated from placebo within months and sustained over years. And the cardiovascular benefit was independent of the weight loss.
That last sentence is the entire story.
In this video, Dr. Trevor Turner — a board-certified PM&R physician and precision longevity practitioner at Pravida Health in Buckhead, Atlanta — walks through what the SELECT trial actually showed, why the cardiovascular and metabolic benefits appear to be independent of weight loss, and how this reframes the longevity case for GLP-1 receptor agonists in patients who aren't using them primarily for weight management.

KEY TAKEAWAYS:
• The SELECT trial enrolled 17,604 adults with cardiovascular disease and BMI ≥27 (no diabetes), randomized to semaglutide 2.4 mg/week vs. placebo
• 20% relative reduction in major adverse cardiovascular events (MACE) — heart attack, stroke, cardiovascular death — over a mean follow-up of 39.8 months
• Cardiovascular benefit was statistically independent of weight loss, separating the longevity case from the obesity case
• Emerging signals suggest GLP-1s may reduce dementia risk and certain cancer incidences — but these are observational and require RCT confirmation
• The honest tradeoff: GLP-1s cause meaningful loss of lean muscle mass and bone density when not paired with progressive resistance training and adequate protein intake
• Patient candidacy for GLP-1 as a longevity intervention should include: comprehensive metabolic workup, body composition baseline (DEXA), and structured resistance training program in parallel
• "Microdosing" GLP-1s is becoming a clinical practice with limited but growing evidence — appropriate for certain candidates, inappropriate for others
• The current standard of care does not yet include GLP-1s as a routine longevity intervention, but the SELECT trial likely accelerates that conversation
QUESTIONS TO ASK YOUR PHYSICIAN BEFORE STARTING GLP-1 THERAPY:
• Has my candidacy been evaluated by metabolic markers (fasting insulin, HOMA-IR, ApoB) — not just BMI?
• Do I have a body composition baseline (DEXA) before starting?
• What is my resistance training plan to protect lean mass during therapy?
• What protein intake am I targeting? (1.6-2.2 g/kg lean mass is the working clinical target)
• How will we monitor for muscle loss and bone density changes over time?
• What is my exit strategy when/if I stop therapy?
ABOUT PRAVIDA HEALTH:
Pravida Health is a precision longevity practice in Buckhead, Atlanta. Our GLP-1 therapy program includes comprehensive metabolic workup, baseline and longitudinal DEXA body composition tracking, structured resistance training prescription, protein optimization, and continuous glucose monitoring during therapy. We treat GLP-1s as one tool in a longevity protocol — not a standalone weight loss intervention.
