All patient information in this video is fictional and created for educational purposes. No real patient records, protected health information, or identifiable data appear in this walkthrough.
In this walkthrough I open the whole chart and code it with you, start to finish. Face sheet, H and P, labs, the operative report, three progress notes, the MAR, and the discharge summary. Then we build the procedure code one character at a time and finish with the coding attestation.
WHAT WE COVER
Why persistent atrial fibrillation is the principal diagnosis on a scheduled admission
How to index each diagnosis in the Alphabetic Index before you verify in the Tabular
Why a resolved pulmonary embolism gets a history code even though the patient is still on a blood thinner
Why long-term anticoagulant use gets its own separate code
Which codes are POA exempt, and why three of them report a 1 instead of a Y
Building the ICD-10-PCS code character by character, and why the root operation is Occlusion and not Restriction or Insertion
The coding attestation, what a coder actually produces at the end of a chart
WHAT IS A WATCHMAN PROCEDURE
In atrial fibrillation, blood pools in the left atrial appendage, a small pouch off the left atrium. Clots form there and can travel to the brain. Instead of lifelong blood thinners, a device is placed through the femoral vein to seal the appendage closed. Understanding what the procedure actually does is how you pick the right root operation.
WHO THIS IS FOR
Anyone studying for the CCS or working toward inpatient coding. Anyone who has looked at a PCS table and closed the book. And anyone who has ever been handed a chart and not known where to start.
ABOUT ME
I'm Coach Tash. Twenty-eight years in inpatient medical coding. CCS credentialed. I have coded and audited in Level 1 and Level 2 Trauma Centers, served as a Federal RAC Audit Contractor for Region A, consulted in 33 of the 50 states, and trained coding teams internationally.
I started with a GED and three kids. If I can do this, so can you.
FREE LIVE CLASS
Thursday, October 1 at 7 PM Eastern. I am putting a real hospital chart on the screen and coding it with you, live.
