A Class 8 engine is built to run a million miles. Plenty do. Plenty of others come apart at 400,000, sitting in a shop waiting on an in-frame that costs more than the truck earns that month.
The difference usually isn't the engine. It's five habits, and none of them happen in one dramatic moment.
Number one is the one that destroys engines while every gauge on the dash reads normal. Wet liners flex with every combustion event, and that vibration collapses vapor bubbles against the liner wall — the same physics that eats boat propellers. Millions of cycles later you've got a pinhole through the liner and coolant in the cylinder. What prevents all of it is additive chemistry you can check with a test strip that costs almost nothing.
