Understanding MRD in Childhood Leukemia | What Does MRD Positive Mean?
Your child's doctor says that the leukemia is in remission, but the MRD test is still positive.
How is this possible? Does it mean the leukemia has returned?
Not necessarily.
When a child is treated for leukemia, doctors examine the bone marrow under a microscope. When leukemia is no longer visible according to the required criteria, the child may be in morphological remission.
However, very small numbers of leukemia cells may still remain. These cells may be too few to be detected by routine microscopy.
This is where MRD (Measurable Residual Disease) testing becomes important.
Using sensitive techniques such as flow cytometry and molecular tests, doctors can detect very small amounts of residual leukemia.
Why is MRD important?
• Helps assess the response to leukemia treatment.
• Helps estimate the risk of relapse.
• Helps doctors make decisions about further treatment.
• Provides important information about the depth of response to treatment.
An important message for parents:
MRD positive does NOT automatically mean that the child has overt leukemia again.
It means that a small amount of disease has been detected, even when the bone marrow appears to be in morphological remission.
MRD results must always be interpreted in the context of the type of leukemia, the amount of disease detected and the timing of testing.
Understanding MRD helps parents better understand their child's treatment journey.
