What is Ozempic face? It's the hollowed-out, older look that can follow rapid weight loss on GLP-1 medications like Ozempic, or after major weight loss from diet and exercise. The fat in your face sits in compartments separated by retaining ligaments. When that fat deflates, the ligaments that were stretched by the extra weight don't shrink, so the skin sags into deep nasolabial folds and jowls, even in patients in their 20s, 30s and 40s. Full article: https://www.drweinfeld.com/blog/ozemp... Austin board-certified plastic surgeon Dr. Adam Weinfeld answers seven questions about Ozempic face: what's happening beneath the skin, what fillers and skin-tightening devices can and can't do after major weight loss, how an extended deep plane facelift releases and resuspends the retaining ligaments, what his TriMax deep plane technique adds, when fat grafting and nanofat help, why the neck often shows weight loss first, and when to schedule a consultation after reaching your goal weight on a GLP-1. What causes Ozempic face? Weight loss doesn't just affect the body; it affects the face. Facial fat is contained in special compartments separated by ligaments, and together they create the shape of the face. When that fat deflates, the stretched ligaments no longer support the skin, and the skin sags. Dr. Weinfeld sees features of aging in rapid weight loss patients that most people don't show until their 50s, 60s and 70s. Can fillers or skin tightening treat Ozempic face? Med spa treatments can add value, but after major weight loss they don't treat the actual problem. Skin-tightening devices tighten the skin, not the loose ligaments underneath it, and skin stretched by years of extra weight can't be tightened enough. Fillers fill, but you can't (and wouldn't want to) refill the face until the skin is taut again. Is a facelift reasonable in your 30s or 40s after weight loss? In Dr. Weinfeld's words, a facelift has nothing to do with age; it's all about anatomy. An extended deep plane facelift reaches the deep tissues through incisions around the ear, releases the retaining ligaments all the way to the nasolabial fold, and resuspends the soft tissue with sutures. What does the TriMax deep plane facelift add after weight loss? Many weight loss patients still have stubborn fat in the jowls and nasolabial folds. With his TriMax deep plane technique, Dr. Weinfeld uses microliposuction in those areas, then recycles that fat: dermal release (subcision) with a little fat in the nasolabial crease, and deeper fat along the bone of the malar eminence for support. Buccal fat reduction is individualized and rarely needed in these patients. How do you lift the cheeks without a pulled look? The lift follows a vertical or vertical-oblique vector, up and out, working perpendicular to the deep folds. Aging moves tissue down and in, so the correction goes up and out, never straight back. Do you need fat grafting after weight loss? Often, yes, in precise amounts in precise areas, such as the malar area against the bone, to support the lifted tissues. Nanofat injected just beneath the dermis is believed to stimulate the deeper dermal cells to create more collagen and elastin. Why does the neck look worse after weight loss? The neck is often the first area to show weight loss, even after 10 or 15 pounds. A blanket of subcutaneous fat can hide stretched platysma bands; when that fat is lost, the bands show, and the loosened skin under the chin can look fuller. It's a paradox: there's less, but it looks like there's more. After major weight loss, a facelift is very rarely done without a neck lift. When should you schedule a facelift consultation after a GLP-1? Come in when you reach your goal weight. Dr. Weinfeld prefers to operate three to six months after goal weight, so nutrition (including 80 to 100 grams of protein, checked with labs such as prealbumin) can catch up and the face can settle.
