The lip is numb.
The tongue is numb.
The block worked.
So why can the patient still feel the tooth?
Profound anesthesia in the "hot tooth" can challenge even experienced clinicians. In this episode of For the Love of Endo, Dr. Brett Gilbert and Dr. William Nudera compare how they approach difficult anesthesia, what they test before starting treatment, and what they do when a conventional block is not enough.
They discuss why a numb lip does not guarantee pulpal anesthesia; how to test soft tissue, the PDL, and the pulp; why simply giving another block may not solve the problem; and when to consider supplemental techniques including PDL, subperiosteal, interseptal, intraosseous, and intrapulpal anesthesia.
They also explore articaine versus lidocaine, epinephrine considerations, paresthesia risk, patient communication, and anesthesia technology.
One of the episode's standout moments comes when Bill describes a subperiosteal injection technique that Brett says he had never used - or even heard of - in 23 years of practice. There is no magic wand for the hot tooth. There is a process, a sequence, and an armamentarium.
The goal is simple: get the patient profoundly numb, keep them comfortable, and know what to do next when the usual approach is not enough. If you like what you're hearing, please like and share to help us grow.
What is your go-to supplemental anesthetic technique when a hot tooth refuses to get numb? Let us know in the comments.
