Not every esophageal emergency is GERD, dysphagia, or simple bleeding. Some esophageal conditions are true emergencies because they reflect ischemic necrosis, full-thickness rupture, mediastinal contamination, or acute upper GI bleeding. In this video from Gastroenterology Made Simple, we explain three high-yield esophageal emergencies in a clear clinical way: Acute Esophageal Necrosis, Boerhaave’s Syndrome, and Mallory-Weiss Syndrome. You will understand: What “black esophagus” means and why it usually affects critically ill patients The two-hit hypothesis of acute esophageal necrosis: ischemia plus acid reflux injury Why upper GI bleeding in shock, sepsis, DKA, or renal failure should raise suspicion for AEN How to diagnose black esophagus endoscopically Why treatment is mainly supportive: NPO, IV fluids, IV PPI, transfusion, and treatment of the underlying cause Why NG tube insertion and acute dilation should be avoided in AEN How Boerhaave’s syndrome differs from Mallory-Weiss tear Why severe chest pain after vomiting is dangerous until esophageal perforation is excluded The meaning and limitations of Mackler triad Why CT chest and abdomen with water-soluble oral contrast is the key first test in suspected Boerhaave The management pillars of esophageal perforation: resuscitation, antibiotics, drainage, closure or sealing, and nutrition When conservative, endoscopic, or surgical management may be appropriate Why Mallory-Weiss syndrome usually causes hematemesis after retching When Mallory-Weiss tear needs endoscopic therapy and when observation is enough Why clips or band ligation are preferred when active bleeding is present This lecture is designed for medical students, residents, gastroenterology trainees, emergency physicians, internal medicine physicians, surgeons, endoscopists, and exam candidates who want to understand esophageal emergencies beyond memorized definitions.
