Autoimmune encephalitides are an important group of potentially treatable inflammatory brain disorders that can present with striking combinations of behavioral change, psychiatric symptoms, seizures, movement disorders, speech or language disturbance, altered consciousness, focal neurological deficits, and autonomic dysfunction. In this educational presentation, we explore a pediatric clinical case of suspected anti-NMDA receptor encephalitis and use it to highlight the broader principles of immune-mediated encephalitis in children. Key topics include: Recognizing the clinical pattern of autoimmune encephalitis Why new behavioral or psychiatric symptoms associated with focal neurological signs should raise concern for encephalitis The significance of MRI abnormalities and the limitations of routine CSF findings Why paired CSF and serum NMDAR-IgG testing is preferred over serum testing alone Major antibody-mediated encephalitides, including anti-NMDAR, LGI1, CASPR2, GABA receptor, and AMPA receptor syndromes The difference between neuronal surface/synaptic antibody disorders and intracellular-antigen-associated, predominantly T-cell-mediated syndromes Genetic susceptibility and the emerging multihit model of pathogenesis Infection-triggered autoimmunity, including post-HSV anti-NMDAR encephalitis First-line and second-line immunotherapy Current areas of consensus, controversy, and ongoing research The central clinical message is: Recognize early. Test correctly. Treat promptly. This content is intended for pediatricians, pediatric neurologists, adult neurologists, residents, fellows, medical students, and other healthcare professionals interested in pediatric neuroimmunology and autoimmune encephalitis.
