99213 – Appropriate E/M code based on the MDM and documentation.
69210 – Removal of impacted cerumen requiring instrumentation.
Modifier 25 – Append to the E/M code if the E/M service was significant and separately identifiable from the procedure performed.
Modifier 50 – Append to 69210 if the payer accepts bilateral reporting with modifier 50. Some payers instead require reporting 69210-LT and 69210-RT, or reporting the code on two separate lines. Always verify the payer's billing policy.
