This symposium explored the dual roles of the pancreas in cystic fibrosis, highlighting how dysfunction in both the exocrine and endocrine compartments contributes to disease burden. It examined the pathophysiology of exocrine pancreatic insufficiency and its impact on digestion, nutrition, and growth, alongside emerging insights into islet cell dysfunction and cystic fibrosis-related diabetes (CFRD). The session emphasized how these two systems are interconnected, sharing developmental origins, structural support, and vulnerability to cystic fibrosis transmembrane conductance regulator (CFTR)-related damage. Recent advances in cystic fibrosis transmembrane conductance regulator modulator therapy and their differential effects on pancreatic function were also discussed.
