Right bundle branch block shows up before the heart thickens in IVS4+919G greater than A Fabry carriers. Conduction disease is the electrical signature, not hypertrophy. 905 carriers from a regional genetic screen showed a 9-fold increased risk of RBBB compared to matched controls, with pacemaker implantation risk also elevated 9-fold. ECG alone detected disease in 88% of carriers when combining LV hypertrophy with conduction abnormalities, substantially exceeding echocardiography alone (51.5% sensitivity). The conduction phenotype precedes structural changes visible on echo, reversing the typical diagnostic sequence. PR interval prolongs rather than shortens, an atypical pattern for Fabry disease. Females showed a catch-up phenomenon after age 60 and carried 2.5-fold higher prevalence than males across age groups, suggesting sex-specific disease trajectories that remain mechanistically unclear. Study confined to East Asian populations where IVS4+919G greater than A is most prevalent. Generalizability to other geographic regions and genetic backgrounds requires external validation.
