I used to think waking up in the ICU on a ventilator meant it was time to start talking extubation. I had no idea the multitude of factors that go into assessing extubation readiness. Our number one goal as an ICU provider should be "When can I get this ETT out of this patient?" And there is a truly systematic/algorithmic pathway to follow to determine this. Now, there is definitely an element of gestalt, it's not always black and white. But overall, this checklist is solid gold, especially when you are new, to use to assess your patients the same way, every time and mitigate the risk/benefit decision.
Join us as my friends and I discuss all things NP while I get ready for work. As always, this is a safe space (albeit public) so please feel empowered to ask what you like and to present an oppositional view if that is your opinion. I welcome all interaction and am generally an open book. I include a topic to discuss as it gets boring real quick without direction so this week it'll be on the cons to this profession.
