Peripartum cardiomyopathy presents in the last month of pregnancy or the first five months after delivery, and it is missed often because the early symptoms — breathlessness, swollen ankles, fatigue — are what every new mother expects to feel anyway.
COVERED
• What peripartum cardiomyopathy is, and when in pregnancy or the postpartum period it appears
• The symptoms that should not be dismissed as normal after childbirth
• How the diagnosis is made: echocardiography, LV ejection fraction, and excluding other causes
• Standard heart failure therapy, and the drugs that must be adjusted while breastfeeding
• Anticoagulation considerations at very low ejection fraction
• Recovery: many women recover LV function, some do not, and what determines which
• Counselling about future pregnancies
Outcomes vary considerably between patients. Early diagnosis and prompt guideline-directed treatment give the best chance of recovery, but no outcome can be promised.
