The full version of my 2 minute pterygoid Reel. Two pterygoid implants placed hands-on on an atrophic maxilla model, start to finish: landmarks, flap design, where to aim, drilling, placement and the multi-unit abutment. The right side ended up engaging the medial plate; the left side popped out between the plates into the pterygoid fossa, so you can see both endpoints side by side. Planning comes first: How To Plan Pterygoid Implants On CBCT    • How To Plan Pterygoid Implants On CBCT   0:00 Intro 0:20 Step 1: Find the tuberosity drop and the GPA 1:04 Flap design 1:39 Pterygoid anatomy: plates, fossa, hamulus 2:06 Step 2: Where to aim 2:31 Danger zones: never go lateral 3:11 Step 3: Drills and handpiece 4:28 Step 4: Starting point and 30° angulation 6:53 Step 5: One 2.0 drill to 20 mm 7:44 Result: it engaged the medial plate 9:18 The implant and a straight driver 10:40 Step 6: Place along the same trajectory 11:00 Dense model bone: widening the top 12:23 Step 7: Equicrestal, then a 30° MUA 13:40 The other side: out between the plates 14:54 Right vs left 15:23 Anatomy recap with both implants in This is a plastic model, and its bone is far denser than a real tuberosity, so the extra widening and the crack you see on the right side are model artefacts. In the mouth I very rarely flare the top. #pterygoidimplant #dentalimplants #implantdentistry
