If you are on hormone replacement therapy and your provider is telling you that you need a Dutch test or serum hormone panel to determine your dose, this video is for you.
I am Dr. Laura Pipher, ND, and this is one of the hills I will die on as an evidence-based practitioner: there is no serum value, saliva value, or urine metabolite that tells us whether your HRT dose is working. Your symptoms tell us that.
I have taken a lot of heat on social media for this position, so in this video I am unpacking the full rationale, including where the confusion comes from and why the pushback is so emotional. Because here is what I am NOT saying: I am not saying we never test hormones in women. There are absolutely clinical scenarios where hormone testing is appropriate and necessary, and I cover those too.
What I am saying is that once you are on menopause hormone therapy, expensive testing is not going to move your care forward. A thorough symptom assessment and a proper follow-up will.
In this video I cover:
-Why testing hormones on HRT can actually work against you, including cases where patients had an effective dose reduced based on an arbitrary number
-The STRAW criteria and how we clinically stage perimenopause and menopause
-Why symptoms plus age (45+) are the diagnostic starting point, not lab work
-When irregular cycles point to something else entirely, like PCOS (now PMOS)
-Vasomotor symptoms as the primary indication for HRT, and why treating hot flashes and night sweats often resolves sleep issues
-How I dose estrogen and progesterone in practice, starting with the lowest effective dose
-Why oral micronized progesterone is standardized at 100mg and what makes us adjust it
-The standardized symptom questionnaire I use to determine whether a dose is working
-Why perimenopause symptoms overlap so heavily with insulin resistance, thyroid dysfunction, low iron, and inflammation, and what I actually do test
-Whether HRT causes weight gain
-The exact questions to ask your provider if they insist on testing your hormones on HRT
-The clinical scenarios where hormone testing IS evidence-based, including premature ovarian insufficiency, post-hysterectomy patients, and pelvic radiation
