The first ten minutes after you sit down decide most of how you will feel when you land six hours later. Almost nobody uses them.
A commercial aircraft at cruise is pressurised to the equivalent of roughly six to eight thousand feet rather than to sea level. For a healthy person that is unremarkable. For somebody with a cardiovascular or respiratory condition, recent surgery, or a circulation problem, that difference is physiologically real.
Add cabin humidity running around ten to twenty percent against the forty or fifty of an ordinary house, and you have an environment that dehydrates people considerably faster than they expect.
That is why those ten minutes matter, because the version of you at cruise is not the version of you at the gate.
