Ok So I haven’t stopped thinking about the topic you may be struggling through quietly and thought that you may like some insight from an old nurse. Been there and done that. Have helped many women accesses, encouraged participation and held hands at appointments. Migraine, Diet, and Reasons for not accessing menopausal support, many levels not just Lower socioeconomic women, I think it's all levels really cause it’s the same issues. I have worked at top hospital, country, outback clinics and more regional hospitals and communities and all very similar issues across the economic levels. Reasons as I see them: · Access, regional women just don’t get the access, Rural people must travel so far to regional centres. City folk have wait lists that are just so long. I heard that allegedly Breast screening was 1 year wait for Hervey Bay. · Communication: they just don’t communicate to GP’s and GP’s have no idea how to simply listen to women’s needs. That also includes lady DR.s. I have heard from so many that GPs are too busy to listen, don’t follow up, don’t believe women, and clients don’t understand what they are talking about. · Too busy: many have a huge social responsibility that has been thrust onto them, such as grandchildren, they have had to bring up and so don’t have time for themselves. · Poor self-esteem. There is so much on this one. Many women from this generation have “closet incidents” they hide, especially rural women. Abuse is rampant for many women of all social levels. Constant knock downs, poor self-belief, no wonder they just gave up caring for themselves. They won’t communicate with people they don’t trust and GP’s are just that, if they don’t trust them and the GP doesn’t take the time to build that trust, nothing will get sorted. · The pecking order: sometimes women are funny they won’t go unless their friends have been or the other way if such and such ash been no way will they do it. In my experience I have seen ladies from rural towns not go to appointments cause the neighbours will see them go and they fear judgement, taunts, ridicule or worse that they have a disease. Women form and various communities I was working in just didn’t follow up on appointments for this reason. If word got out in the community they would be condemned, black banned as they had a disease like leprosy. I’ve actually snuck ladies through clinics and hid them just to make their appointments or taken them in my car and gone in with them just to hold their hands and advocate for them. · Body Shaming: for many years they have been condemned about our bodies, whether they are good looking, skinny, medium, big or tubbers. That poor self-esteem kicks in when someone says one little thing, not even meaning anything, but that old pattern of worthlessness, chronic doubt, who cares anyway, just kicks in. then comes the stress and social pressures of diet, eat this, eat that, don't eat this, don't eat that. Its all a bit much really. Some of the group diet models from a few years back, literally cause so many body, diet and social esteem issues. I am glad they are off the scene now. the food we eat needs to be sound and nutrious, good wholesome foods, we grow, we make and learning what's right for each individuals body. But then if we do stray, we should not condemn ourselves for having some junk food. on occasions. · Money: how will they pay for scans and tests that aren’t on Medicare. 4 to 500 for a MRI. How will they get to places if they don’t have a car or someone, they trust to take them to the appointments. Medications: how are they going to afford the extra non scripted medications More immediate concerns such as chronic diseases, mental health, or family health may take priority. Working in rural towns you saw the great in people and then the worst. and woe be tide a lady if they were on the outer. It’s that schoolgirl mentality, gossip and stigmatizing people comes out worst. I also see a lot of the old ladies as gossip queens and the secret squirrel taunts are quite astounding. Sexually abused women are special, It's time and trust building. GPs just don’t get it. I’ve worked with quite a few women over my time and all the same, it takes time, consistency, and communication that is simple and believable. Loads of self esteem work, worthiness work and self-forgiveness. It takes time. Trust, love and caring. Eventually you may get through. Holding hands, advocating and being present with these ladies is always helpful. Brings a sense of acceptance and self-importance, self-worth. I normally bring some form of Reiki, simple and achievable goals. Listening and praise. Acceptance of all achievements and failures. Anyways I love a good topic, thinking and sharing. Well that was a bit of a topic. I don't confess to be a expert. I just experienced things through my nursing career
