What actually happens if adults eat the equivalent of 1 cup of blueberries every day for six months? The answer is more interesting than the usual “blueberries lower blood pressure” headline. In a 6-month double-blind randomized controlled trial involving 115 adults with metabolic syndrome, researchers compared blueberry intakes equivalent to 1 cup per day, 1/2 cup per day, and placebo. The 1-cup group improved flow-mediated dilation — a measure of how well blood vessels respond and widen — by about 1.45 percentage points. But insulin resistance and blood pressure did not significantly improve. The 1/2-cup intervention did not produce clear improvements in the measured biomarkers. That creates the real question: If blood pressure barely changed, what exactly improved inside the blood vessels? In this FOOD INQUEST episode, we examine endothelial function, flow-mediated dilation, blueberry anthocyanins, circulating polyphenol metabolites, dose-response evidence, and why vascular function and blood pressure are not the same measurement. We also look at newer pooled evidence. A 2024 systematic review included 16 blueberry studies, with 11 included in the meta-analysis. Across the trials, blueberry interventions improved flow-mediated dilation by about 1.50 percentage points and reactive hyperemia index by 0.26. Diastolic blood pressure fell modestly by about 2.2 mmHg, while the overall systolic blood-pressure effect was not statistically significant. Another randomized human experiment found that blueberry polyphenols improved vascular function within hours. The response increased with dose up to roughly 766 mg of blueberry polyphenols and then plateaued rather than continuing to rise. And in a 12-week trial involving healthy adults aged 65–80, freeze-dried wild blueberry powder equivalent to about 178 g fresh blueberries improved flow-mediated dilation by about 0.86 percentage points and reduced 24-hour systolic blood pressure by about 3.6 mmHg compared with placebo. The key takeaway: Blueberries do not behave like a blood-pressure drug. The more consistent human signal appears to be improvement in certain measurements of endothelial and vascular function. The exact effect depends on the population, blueberry preparation, dose, duration, and outcome being measured. SOURCES / FACT CHECK 1. Curtis PJ et al.
